The Arizona Health Care Cost Containment System (AHCCCS) said more than 3,000 individuals who receive Medicaid coverage may have had some personal information compromised.
The agency said an investigation found 3,146 IRS 1095-B forms were misdirected to incorrect home addresses due to a programming error.
The form is used to report an individual’s enrollment in a qualified health plan. AHCCCS says the form contains names and dates of birth, but not Social Security numbers.
The agency suggests that those affected check their credit scores, and place fraud alerts or security freezes if need be. They are also advised to contact local authorities if there’s any suspected identity fraud.
